It is your responsibility to know the specific requirements of your insurance, such as when you need a referral, a prior authorizaiton, and which specialist your plan allows you to see. Questions about coverage and benefits under your policy should be directed to your insurance carrier.

Private pay or self pay patients should plan to pay for professional services at the time they are rendered. This helps keep your costs down by eliminating the expense of monthly billing. We offer a 20% discount for self pay patients when they pay in full at the time of service. Four your convenience, we accept cash, check, or credit cards.
